Sick notes are only required for absences from work longer than 7 days. Please make an appointment to see a doctor if you require a sick note.

We can occassionally issue a sick note without seing you  based on a hospital discharge or a report. Contact the surgery if you feel this may apply. 

Absences of less than 7 days are covered by self-certification.
