CORONAVIRUS:


Call 111 now if you've been:





      • to Wuhan or Hubei Province in China in the last 14 days (even if you do not have symptoms).

      • to other parts of China, including Macau and Hong Kong, in the last 14 days and have a cough, high temperature or shortness of breath (even if it's mild)

      • to Thailand, Japan, Taiwan, Singapore, Republic of Korea or Malaysia in the last 14 days and have a cough, high temperature or shortness of breath (even if it's mild)

      • in close contact with someone with confirmed coronavirus




Do not go to a GP surgery or hospital. Call 111, stay indoors and avoid close contact with other people.


Tell 111 about any recent travel and any symptoms you have

ACCESS TO MEDICAL RECORDS

Online access to your medical records

we are offering patients online access to their medical record. This access allows you to make appointments, request medication and also gives you full access from the day you register to your medical records which includes documents, blood results and consultations.

If you would like to take advantage of this service then please read the information below, print off the necessary forms and bring them into the surgery with photo ID.

If you have not already got online access a receptionist will print you a pin number so you can register online.

You will need to fill out a questionnaire and disclaimer to view your full records and allow 10 working days for it to be activated.
